 500 Error not showing in logs?

We're getting 500 errors (seemingly at random) when clicking the second instance of checkout button. The strange thing is they are not showing up in the logs. It started two days ago apparently and yesterday I encountered a couple doing some test orders but couldn't find any reference to them in the logs. I was able to find some errors from the previous day and I opened a ticket with the host who told me they had something to do with duplicate coupon codes even though there were no coupon codes being used. I'm on Version 7.5.2.

I discovered this error and reported to Vince and there is a new update today to fix this very issue.
Strangely enough, I also did not have the error in my logs.

PS - For me, it occurred when adding more than one (unique) item from a sigle category that had a coupon assigned.
