Is there a way to add an option for people shipping outside of the USA to add insurance to their order? If yes, can we charge a fixed rate?

These are the shipping insurance options https://www.ecommercetemplates.com/phphelp/ecommplus/parameters.asp#shipin - you can set a fixed amount using the shipinsuranceamt parameter. There isn't a feature to make it country specific though, if that's what you were suggesting in the question.
