1 - Mark the item(s) shipping exempt in the product.
2 - Create a required product option which I called Shipping Destination.
3 - Offer 2 options:
US Delivery with no charge

International Delivery with an appropriate charge (Will be a flat rate, so calculate shipping appropriately)
You could actually do multiple destinations with different pricing

4 - Add this option to the product(s) you want shipping exempt to a specific area.
