Is there a built in ECT form that can post the data to to MySQL or a CSV file and the fields also be manipulated/changed. Or an online simple form that you recommend to do this.

There is a built in (csv) utility for bulk changes to products and images.
There are a number of 3rd party cvs plugins for other data.

I just need a contact form with address, name, email and some additional fields. Phil hooked me up with a cool form that emails the information, but i need it in a CSV file or such because we are going to actually UPS/USPS them some samples and I don't want to go through 100's of emails to gather the mailing address. Unless there is a simple email parsing tool. I would think this would be an easy task to do for an experienced developer.
