After updating to version 7.6.5 Cart.asp does not show fields for the customers name, address, city or phone number. A screen shot is at https://mindamusicstore.com/checkout.jpg

How do I correct this?

It looks like you're missing the line below on cart.asp

<script type="text/javascript" src="/js/ectcart.js"></script>

Make sure it's added to all the other store pages as well and you uploaded the latest version from the updater to the js directory

Could have sworn I'd done that. Obviously not.
