Its been a while, but I fired my old SiteSurvey software again. I created a survey and embedded it into a webpage outside of the sitesurvey folder. I reset the path to the db and the survey includes files. Everything seems to work except for...

After completing the survey and clicking Submit I get a 500 server error. Everything seems to be set correctly. What am I missing?

An error of 500 is a generic server error, and it could be almost anything. You will need to find out the actual error, in the the server error logs.
