I noticed we haven't gotten any UPS shipments since September, and found that it is giving "Missing/Illegal ShipTo/Address/StateProvinceCode" now for orders outside Canada.

It works if I switch to Published Rates but not Negotiated Rates.
Checked the Shipping Admin and all the same UPS methods are still selected.
I tried both "Update Account" and "Re-Register" on the UPS Developer Kit Licensing & Registration Wizard. No change. I'll note that there was an error "Unknown column 'adminUPSUser' in 'field list'" when finishing the wizard, but it did still say Registration Successful.

What's up?

It's always been a requirement for some reason that with Negotiated Rates and when using UPS you need to set the origin state as well as the origin zip. This is done by setting the parameter $defaultshipstate in your includes.php file. Is it possible that that was removed somehow?

This was fixed in a recent update but before re-registering, try looking for the $defaultshipstate and if that doesn't sort this, add this parameter to your includes.php file,
dumpshippingxml=TRUE;
...then go through checkout and you should get the XML call and response displayed. Then copy and paste that to me in an email (vince AT ecommercetemplates DOT com)

Same as it has always been.

Emailed you the XML. I notice this line in the UPS section: <StateProvinceCode></StateProvinceCode>
Possibly the issue, but not sure why Indiana/IN isn't being passed through?

I've been doing some testing here and the US state abbreviation seems to be filled in as it should. Could you try applying one of the latest updaters and see if that helps, and if not then could you send me the site FTP login to my email and I'll see what is up.
