It's possibly a scenario where the customer submits the order and the payment acknowledgement is slow to come in, and while that's happening the order can actually be changed by the customer. I (if that's what happened) expect the customer removed the items. Have you spoke to the customer, and what payment provider is this?
From time to time, we have customers change or remove contents of the cart when paypal IPN's are slow to come it.
While this may not be your issue, it's certainly a possible explanation.
