When registering for a new client login account, how does the customer say whether it is a regular account or a wholesale pricing account that they want?

It's not something they can set themselves as otherwise anyone could just set themselves up with an account and get wholesale pricing. You have to add the attribute in the client login admin side.
You can however set the default when they register, and that is explained here...
