add custom note to a particular item

For example I sell items that are custom made to the customers specifications. Id like the customer to be able to enter his/her exact length and width. Now I have to email them and go back and forth. Im looking to stream linethis.

How long has that "text only" button been there? Ive been using ecomm for 13 years and this is the first time, Ive clicked it! Dave I cant do it that way, there would be thousands of option values.
